Change of Address Checklist

When moving, send a change of address information card to all that apply from the list below. If you know the date you will be moving - send the cards in advance with your moving date and the date they can start sending things to the new address. This will eliminate much of the hassle of forwarding mail - and the chance you might not ever get it. To make it easier on you - have pre-printed cards made (you can do it on your computer) that you can just slip in with your payments or into an envelope. This way nobody can misread your writing and input the wrong information.

• Charitable Organizations
• Church
• Clubs
• Credit Card Accounts
• Creditors
• Food home delivery services (Milk, cheese, ice cream, meat, etc)
• Dentist
• Doctors (primary care and specialist)
• Employer
• Friends
• Landscape Company / Gardener (or cancel service if they don't work in the new area)
• House Cleaning Services (or cancel service if they don't work in the new area)
• Insurance Companies (Car / House / Business)
• Internet Provider (or cancel service if they won't transfer current account information)
• Legal Business Forms if you own a business and work out of your home.
• Motor Vehicle Dept (this must be done in person for address change)
  While you are there, remember to register to vote if you are moving to a new county.
• Magazine Subscription (if you have any, cut your address label from magazines and send with your address change card - this has your account number on it and will make the change easier)
